Graduate Fellowship Recipient Exception Form

If, due to extenuating circumstances, you must deviate from compliance with fellowship regulations, please complete the Graduate Fellowship Recipient Exception Form as follows:

1. Complete Part I of the form.
2. Obtain departmental support and the appropriate signatures in Part II.
3. For Graduate School Fellowships or Dissertation Fellowships: Return the completed exception form to the Graduate Fellowship Office, Mitchell Hall 251, or mail it to the following address:
